Tatizo la Mtindio wa Ubongo au Kupooza ubongo kitaalamu ni Cerebral plasy (CP), ni hali ya kupooza ya moja kwa moja ya viungo vya mwili inayotokana na sehemu ya ubongo (seli) inayotawala viungo hivyo kufa katika kipindi cha mwanzo cha maisha (Utototni).

Mtindio wa Ubongo ni aina ya tatizo la mishipa ya fahamu inayoathiri misuli ya mwili inayotokana na selli za ubongo wa mtoto kufa au kutengenezeka vibaya katika kipindi cha mwisho wa ujauzito (3rd trimester), kipindi cha kuzaliwa au kipindi cha miaka miwili ya mwanzo wa mtoto.

Chanzo chake

Hii inatoka na sehemu ya ubongo kukosa oksijeni na virutubisho vya kutosha inayopelekea celli hizo kufa. Tunajua kuwa ubongo una kazi mbalimbali mfano kufikilia,kufanya maamuzi(decion making) pia inatawala sehemu mbalimbali ya viungo vya mwanadamu,mfano ubungo wa nyuma unahusika na kuona kama wakati mtoto akichelewa kulia cell za ubongo wa nyuma (occipital) zikafa mtoto huyu baadae atakuwa na matatizo ya kuona.

Vitu vingi vinaweza kusababisha ubongo kupooza, navyo ni katika kipindi cha ujauzito (mtoto akiwa tumboni), wakati wa kujifungua(80%) na kuugua mtoto kipindi cha mwanzo kabla ya miaka miwili.

Kuchelewa kulia baada ya kuzaliwa

Zaidi ya 67% ya watoto wote wenye tatizo la ubongo kupooza(CP) Tanzania inatokana na kuchelewa kulia baada ya kuzaliwa(birth asphyxia) ambayo inatokana na mama mjamzito kukaa na uchungu kwa muda mrefu(prolonged labour).

Manjano na degedege

Ikifatiwa na manjano (severe neonatal jaundice) na degedege baada ya kuzaliwa inayopelekea celli za ubongo kufa.

Matatizo yoyote yanayoweza kuathiri ubongo

Kabla ya miaka 2, mtoto akipata homa ya uti wa mgongo(meningitis) mtoto kuanguka(head trauma), malaria kali, matatizo haya yanaweza kuathiri seli za ubongo za mtoto ambazo haujakomaa.

Matatizo wakati wa ujauzito

Katika kipindi cha awali ujauzito(1st trimester) mama akipata mashambulizi ya magonjwa na kipindi cha miezi ya mwisho ya ujauzito (3rd trimester) shinikizo la damu kuwa juu (Pregnancy Induced Hypertension) na kifafa cha mimba/eclampsia kinaweza sababisha usafirishaji wa damu yenye oksijeni na virutubisho kupitia kondo la nyuma kushindikana vizuri hii inaweza pelekea baadhi ya cell kwenye mwili wa mtoto aliye tumboni kufa.

Matatizo katika kipindi cha kujifungua

Kwa utafiti uliofanyika katika hospitali ya taifa ya Muhimbili mwaka 2013 kuhusiana na sababu za watoto waliopooza ubongo, inaonesha zaidi ya 80% ya watoto wenye tatizo hilo linatokana na matatizo katika kipindi cha kujifungua.

Matokeo/madhara.

Madhala ya ubongo kupooza huwa yanatofautiana kutoka mtu mmoja hadi mwingine kwasababu inategemeana na sehemu ipi ya ubongo iliyoathiriwa zaidi.

Kupooza ubongo(CP) kunaleta matokeo mabaya ya moja kwa moja(permanent) katika mwili wa mtoto, Kila mfumo katika mwili wa mtoto unaweza athiriwa mfano mfumo wa chakula, wa hewa, mkojo,fahamu, na wa misuli ,athari zenyewe ni kupata degedege, Kifafa, mtoto kuwa na kichwa kikubwa(macrocephaly), mtoto kuwa na kichwa kidogo sana(microcephaly), tatizo la kusikia, kuona, kuongea,kumeza pia kushindwa kutembea na kupata choo ngumu (constipation) , Pia utahila (mental retardation).

Matibabu ya Ugonjwa wa Mtindio wa ubongo

Tatizo la ubongo kupooza hakuna matibabu yake, kinachotibiwa ni matokeo ya tatizo hili yanayosaidia kubolesha maisha yao na kuwa na hali ya kujitegemea.
Katika matibabu kuna aina mbili nayo ni medical (dawa) na mazoezi(therapy).

Dawa zinazotumika mfano phenobarbitone na Carbamazepine kwa degedege na kifafa na kukakamaa (contracture,spasticity) hupewa BOTOX inayosaidia kupunguza tatizo.

Kuna therapy mbalimbali kwa watoto wenye CP, nazo ni Physiotherapy, Occupational therapy, Speech therapy n.k.

Kwa tatizo la kutoweza kutembea, watoto hawa wanaweza pata faida katika kitengo cha mazoezi(physiotherapy) mzazi anaweza kufundishwa ili kumsaidia , misuli ya mwili kupata nguvu,hupunguza kukakamaa na pia inasaidia kupata ujuzi wa kutembea n.k. Pia kuna Occupational therapy inamsaidia mtoto apate uwezo wa kula mwenyewe na kukaa.Speech therapy/kuongea, watoto wengi wenye CP huwa na tatizo la kuongea hii inatikana na misuli inayosaidia katika kuongea kuathiriwa, kuna vifaa vinavyotumika kumsaidia mtoto mwenye tatizo hili ili aweze kuongea vizuri.

Sio watoto wote wenye kupooza ubongo wana utahila (mental retardation), 36% ya watoto wenye ubongo kupooza wana utahila(mental retardation). Mtoto anaweza kuwa na tatizo la viungo ila ana uwezo mkubwa wa kufikilia.

Matokeo kwa jamii na familia.

Jamii:Kwa utafiti uliofanyika katika hospitali ya Muhimbili, inaonesha zaidi ya asilimia 30 ya wazazi wanahusisha tatizo hili na mambo ya kishirikina(superstition) Wazazi wengi wenye watoto hawa wanafanya maamuzi ya kuwaficha watoto wao vyumbani kutokana na mtazamo wa jamii husika .

Familia:Pia imegundulika kuwa ndoa nyingi huvunjika mara baada ya kuzaliwa watoto wenye CP, kati ya wanawake 100 wenye watoto hawa waliohojiwa katika hospitali ya Muhimbili 16 wanaishi peke yao baada ya kupata watoto hao, wengi wao wanadai kuwa waume zao wanawalahumu kuwa wenyewe ndio chanzo cha hao watoto.

Pia wanawake wengi hupata changamoto katika malezi ya watoto hawa, wengi wao huwatekeleza katika vyumba inayopelekea kuharibika kisaikolojia.Hii inatokana na hali ya uchumi wa familia, mama na wanafamilia wote kuondoka nyumbani kwenda kufanya ujasiliamari ili kujikimu na maisha.

Familia :upendo unahitajika wa hali ya juu kwa watoto hawa, ili wasiharibike kisaikolojia. Kuna watoto wameweza kufanya vizuri katika masomo ambao wana tatizo hili, sio wote wanapata utahila, mtoto anaweza kushindwa kutembea lakini akawa na IQ kubwa, katika watoto waliokuwa wanahudhulia clinic ya watoto wa CP na kifafa Muhimbili, alikuwepo mmoja aliyekuwa anaongoza katika darasa analosoma na pia anamipango wa kusoma ili kuwasaidia watoto wenye tatizo hili, pia kuna madaktari bingwa wa mishipa ya fahamu kwa watoto walioko marekani(Paediatric neurologist) ambao wana matatizo hayo.

Wazazi wasikate tama kwa watoto hawa kwa sababu kwa therapi mbalimbali watoto wanaweza rudi katika hali kama watoto wengine, pia wakiendelea kutumia dawa na kuhuzulia clinic kama watu wenye sukari na presha.

Ushauri kwa viongozi serikali

Uongozi: Kupitia vyombo vya Habari na vipeperushi , jamii ya Tanzania katika miji na vijiji, elimu itolewe kuhusiana na sababu ya tatizo ili kuweza ziepuka. Pia kuna vituo vichache sana vya mazoezi (physiotherapy na occupational therapy) wazazi wanapata changamoto nyingi, katika jiji la Dar, wengi wao huenda katika hospitali ya taifa Muhimbili kwa ajili ya mazoezi, kutokana na hali ya uchumi, nauli inakuwa changamoto wanaamua kuacha kuhudhulia kliniki.

Fursa ya kufungua vituo vya mazoezi(physiotherapy) na shule katika kila wilaya kwa sekta binafsi na serikali itasaidia watoto wenye CP. Pia shule za watoto wenye CP katika nchi ya Tanzania hazipo, kuna shule chache za watoto wenye utahila, watoto wenye CP sio wote wenye utahila.
